BCFO provides Lymphedema garments through area medical supply providers. To qualify, candidates must be uninsured or underinsured, and lymphedema must be a result of breast cancer treatment.

1.) Fill out the Lymphedema Garment application and Medical Records Authorization and fax, mail or drop off at BCFO office: BCFO, 620 W. Republic Rd., Ste. 107, Springfield, MO 65807

2.) Please be specific about what type of garments are needed.

3.) BCFO will request a copy of the most recent medical records related to your need for lymphedema garments

4.) Once all necessary information has been received, applicants will receive a letter regarding approval with instructions on how to proceed.

Payment for Lymphedema garments will be paid to invoice to the garment provider.
